demonstrate your talents, and perhaps, secure a career position. We want your
experience as an employee of Martha Vance & Associates to be positive and
productive.
Remember, although you may be assigned to work for our client and they will
direct your work, you remain an employee of Martha Vance & Associates. If
you have any questions regarding your employment or are offered a position by
our client, even if it is for a different position than you are employed as a
temporary, be sure to contact your personnel consultant immediately.
GENERAL BACKGROUND

Job Satisfaction
Your job satisfaction is very important to us. The happier you are with your
work, the more productive youll be. Please contact your personnel consultant
immediately if your job duties differ substantially from those initially
described to you or if you are not happy with your current assignment.
Attendance and Punctuality
One of the best ways to build a good work reputation is to be reliable and to
report to work on time. Always be punctual. Impress your employer with a
conscientious effort to honor your commitments. It may lead to future
assignments and even a permanent position with the company for whom youre
working. Attendance is a key element and is essential for success in your job.
Absenteeism and tardiness place a burden on other associates, on the Client
Company, and on Martha Vance & Associates.
In those rare instances when you cant avoid being late or are unable to
work, notify your personnel consultant as soon as possible and they will contact
the Client Company.
Poor attendance or excessive tardiness may jeopardize future assignments or
result in termination of employment.
Personal Appearance
Your personal appearance is part of your professional image. The way you
dress and conduct yourself will contribute to the high business standards of
Martha Vance & Associates and its client companies. When on assignment,
dress in appropriate business attire, and present a professional image
consistent with the standards set forth by the Client Company.
Benefits
Paid Holidays
After completing 1,000 hours of employment with Martha Vance &
Associates, you are entitled to the following six paid holidays - New Years
Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas
Day. To qualify for the paid holiday, you must be on assignment the day before
and the day following the holiday. You will receive 8 hours compensation at your
current hourly pay rate for each paid holiday.
Vacation Pay
Upon completing 1,800 hours of temporary employment within two years of your
first assignment, you are eligible for 40 hours of Vacation Pay. This paid
benefit is based on the average hourly rate you earned for the first
1,800 hours worked. Each time you accumulate 1,800 hours, you will qualify for
vacation pay benefits.
Retirement Benefits
Temporary associates become eligible for participation in Martha Vance &
Associates Retirement Plan after having worked two consecutive years.
Associates must also earn a minimum of $5,000 in each of the two consecutive
years to be eligible for participation.
Referral Bonus Program
Earn unlimited CASH BONUSES with Martha Vance & Associates progressive
Referral Bonus Program. Tell your friends about our service, and if they work on
a temporary basis for just 80 hours or are placed in a direct hire position, well
send you a Cash Referral Bonus. Recommend us to as many people as you desire.

Training Opportunities
Self-paced, PC-Based training is available to all associates. Learn new
skills or brush up on specific tasks with the most recently produced software
packages including:The software suite of products from Microsoft, Corel, Lotus
and Quickbooks. You may also practice and improve your skills in typing, data
entry and ten key.
Family and Medical Leave Policy
During your employment with Martha Vance & Associates, you may become
eligible for unpaid leave under the Family and Medical Leave Act of 1993 (FMLA).
This law entitles eligible employees to take up to 12 weeks of unpaid,
job-protected leave in a 12-month period for specified family and medical
reasons.
To be eligible for FMLA benefits, you must:
- work for Martha Vance & Associates for a minimum of 12 months, and
- have worked at least 1,250 hours during the 12 months prior to the beginning of your leave.
If you are eligible, you will be entitled to take a total of 12 work weeks of
unpaid leave during a specified 12-month (leave) year, BUT ONLY
for one or more of the following reasons:
- for the birth and care of your newborn child
- for placement with you of a child for adoption or foster care
- you are needed to care for an immediate family member such as a spouse,
child, or parent with a serious health condition, or if you are unable to work at all or are unable to perform any one of the essential functions of your position because of a serious health condition.
Please feel free to inquire with respect to your FMLA rights and
responsibilities with your personnel consultant at any time. A copy of the
Martha Vance & Associates FMLA policy is available upon request. You will
also receive notification of your FMLA rights and responsibilities each time you
request FMLA leave.

Introduction
Since 1979, Martha Vance & Associates has been and industry leader in the
employment business. All our policies and methods have been time tested
by real people in real situations.
Getting paid is easy. Just follow these simple procedures and youll be on
your way.
Office Hours /Availability Line
Your time is valuable. We respect that, so keep us up to date on your
availability for temporary assignments or career opportunities. Contact our
Availability Line 24 hours a day, 7 days a week and let us know when you are
ready for employment.
Regular business hours are 8:15 AM to 5:00 PM Monday through Friday, Eastern
Time.
Our office does have voice mail and you may leave a message 24 hours a day, 7
days a week.

Job Assignment
Your personnel consultant will contact you when there is an assignment that
matches your employment objectives. At that time, all the relevant information
you need will also be discussed, such as job responsibility, location, pay rate,
duration of assignment, and appropriate dress. You have the option to accept or
refuse any assignment. When you do accept an assignment, we expect you to
complete it to the best of your abilities.
Workweek and Paydays
Our workweek begins on Monday and ends on Sunday. You will be paid each week
according to the time card you send us that verifies the days and hours you
worked. All associates are paid weekly. Each paycheck will include earnings for
work performed through the end of the previous payroll period. Overtime will be
paid for any time worked exceeding 40 hours in the 7-day workweek. The overtime
rate is time and one-half.
Hourly pay rates vary by assignment. When you accept an assignment, you also
accept the pay rate and terms of the job quoted. Martha Vance & Associates,
in conjunction with our Client Companies will determine if and when pay
increases are appropriate. Your personnel consultant will coordinate any changes
in your rate of pay with the Client Company. In the interest of privacy, please
do not discuss your pay with clients or co-workers.
Filling out Time Cards
Properly preparing time cards pays off. Youre provided with a 3-part time
card with a return address mailer. Use one time card for each workweek and
for each assignment. Filling out your time cards accurately and returning them
to Martha Vance & Associates on a timely basis will prevent any delays in
receiving your paycheck. At the end of the workweek, its important to sign
your time card and have your immediate supervisor sign it, too. Both
signatures are required for payment disbursement. Leave the top, white copy of
the time card with your supervisor, keep the pink copy for your files, and mail
the card stock copy to us using the return address on the opposite side. You may
also use our time card drop box located just outside our front door. Time cards
must be received by Tuesday to be included in Wednesdays payroll processing.
Incomplete, illegible, unsigned or faxed copies of time cards cannot be accepted
for processing.
Remember, we have a printable copy of our timecard on our web site. Feel free
to use copies of this document if you find you do not have a timecard copy.
Check Pick-up
Payroll checks processed on Wednesday may be picked up in our office on
Thursday during regular business hours. Simply mark the appropriate box on the
front of the return address mailer of your intention to pick up your paycheck on
Thursday.
Direct Deposit
Paychecks are available for Direct Deposit with your financial
institution if we receive a bank deposit slip or voided check for processing.
Direct Deposit privileges usually begin two weeks after we receive your bank
deposit slip. Associates will receive an itemized statement of wages each time
Martha Vance & Associates makes a direct deposit. Direct Deposit funds are
available at 12:01 am the Friday following Wednesdays payroll processing.
Payroll Overview
- Timecards are due Tuesday to be included in Wednesdays Payroll Processing.
- Paychecks not held for pick up are mailed Wednesday
- Paychecks held for pick up are available between 8 & 5 on Thursday
- Direct deposit funds are available at 12:01am Friday
Personal Data
Its important to promptly notify your personnel consultant of any changes
in your personal data. For example: personal mailing address, telephone numbers,
names and number of dependents, and emergency contact information. Please help
us keep all your personal data accurate and up-to-date.
To properly process your payroll information, you are required by law to
complete the following documentation: Federal Form I-9 with copies of a
passport, drivers license and birth certificate or social security card,
Federal Form W-4, State of Ohio Tax Form, . These forms must be completed with
copies of the necessary identification documents and in our possession before
any pay can be generated.

On the job procedures have been established to protect you, the Client
Companies, and Martha Vance & Associates. The information provided here is
vital to your safety and success. Please listen carefully.
Offers of Employment
Many of our temporary associates are offered employment by our clients, and
were delighted when that happens. Please notify your personnel consultant
immediately if you are offered a position.
Confidentiality
The protection of confidential business information and trade secrets is
vital to the interests and success of Martha Vance & Associates and its
Client Companies. Such confidential information includes, but is not limited to,
the following examples:
- Computer processes, programs and codes
- Customer lists and preferences
- Financial information
- Marketing strategies and research
- Pending projects and proposals
- Research and development strategies
- Technological data and prototypes
You may be required to sign a non-disclosure agreement as a condition of
employment. Those who improperly use or disclose trade secrets or confidential
business information will be subject to disciplinary action, up to and including
termination of employment.
Sexual and Other Unlawful Harassment
We are committed to working with companies where the work environment is free
of discrimination and unlawful harassment. Unwelcome actions, words, jokes, or
comments based on an individuals sex, sexual orientation, race, color, age,
religion, national origin, ancestry, disability or any other legally protected
class, category, or characteristic will not be tolerated. For example, unwelcome
sexual advances, requests for sexual favors, and other verbal or physical
conduct may constitute sexual harassment.
MARTHA VANCE & ASSOCIATES WISHES TO BE COMPLETELY CLEAR. HARASSMENT WILL
NOT BE TOLERATED, AND EMPLOYEES PARTICIPATING IN SUCH ACTIVITY ARE SUBJECT TO
TERMINATION.
If you believe that you have been subjected to harassment on the basis of
sex, sexual orientation, race, color, age, religion, national origin, or
disability, by any Client Company employee, by an individual associated with the
Client Company, or with whom the Client Company does business, you should report
such conduct. The report may be made to any one at Martha Vance & Associates
with whom you feel comfortable.
All complaints will be investigated and appropriate disciplinary action will
be taken when warranted.
All complaints and information regarding any investigation will be maintained
in strictest confidence to the greatest extent possible. No retaliation will be
taken against any employee making a legitimateharassment complaint.
Drug and Alcohol Policy
Martha Vance & Associates is committed to providing a safe, efficient,
and productive work environment free from the usage of illegal drugs and the
abuse of alcohol. Possession, consumption, or abuse of either alcohol or illegal
drugs while on assignment at a client company will not be tolerated, and may
subject an employee to dismissal.
If Martha Vance & Associates or its Client Company has reasonable cause
to believe that an employee is engaging in the illegal use of drugs or the abuse
of alcohol while on assignment, the employee may be required to submit to
appropriate substance testing. A positive test result or refusing to submit toa
test may subject the employee to immediate termination. You may obtain
additional information regarding this policy from your personnel consultant.
Safety Issues
Martha Vance & Associates and its Client Companies are committed to
providing safe working environments free from potential hazards. We urge you to
request specific safety instructions related to the clients work site. In
general, use good, safe judgment when performing your job.
If you feel uncomfortable performing any function of your job due to an
unsafe working environment, contact your personnel consultant immediately.
Additionally, associates are NOT permitted to handle cash or securities or
operate any motorized vehicles or equipment.
Ergonomics
Many assignments require you to work on a computer. The following overview
demonstrates the proper way to position your body while seated at a computer.
- For proper posture, keep your back straight and your neck directly
above your hips.
- Position your monitor at eye level, and at an arms length away from
your body.
- Keep your wrists above a 15-degree angle while working on a
keyboard.
- Keep your back and upper legs at a 90-degree angle.
- Keep your feet flat on the floor.
- When lifting an item off the floor, keep ears, shoulders and hips
in a straight vertical line.
